Hospital Highlights • Type of Facility: Acute Care Facility • Scrub Color: Scrubs or Business Casual Key Responsibilities • Complete intake and admission screenings. • Participate in the development of patient treatment plans. • Perform comprehensive bio-psychosocial assessments. • Provide patient and family education, support, and advocacy. • Collaborate with a multidisciplinary team to ensure continuity of care. • Conduct Case Management and Utilization Review activities. • Manage discharge planning and coordinate patient transitions of care. • Provide weekend coverage for up to two units, with a focus on discharge planning. Required Experience • Minimum 2 years of acute hospital case management experience required. Schedule & Coverage Requirements • Weekend Requirement: Every other weekend. • Must be available for holiday coverage. • Expected to cover up to two units on weekends and prioritize discharge planning. Licensing Requirements • California State License: Preferred. • Pending License Accepted: Yes.
