Unity Home Health is hiring a RN/LPN Field for home visits in the following areas:

Eunice, Opelousas, Sunset, Carencro

We also have an opening for a weekend only PRN RN.

Key Responsibilities • Patient Care: Administer medications, perform wound care, and monitor vital signs. Provide education to patients and families about health management and care procedures. • Assessment and Planning: Conduct comprehensive assessments of patients' health conditions and develop individualized care plans in collaboration with healthcare teams. • Documentation: Maintain accurate records of patient progress, changes in health status, and care provided. Ensure compliance with agency policies and state regulations. • Collaboration: Work closely with physicians, therapists, and other healthcare professionals to coordinate care and ensure optimal patient outcomes. • Support and Guidance: Offer emotional support and guidance to patients and their families, helping them navigate their health challenges and treatment options.

Qualifications • Education: Must be a graduate of an accredited nursing program and hold a current, unencumbered RN license. • Experience: One or more years of clinical experience as a Registered Nurse, preferably in home health or a related field. • Skills: Strong clinical skills, excellent communication and interpersonal abilities, and the capacity to work independently in various home environments.

Work Environment • Home Health RNs travel to patients' homes, providing care in diverse settings. This role requires flexibility and adaptability to different home environments and patient needs.
