PACE Licensed Dietitian – Part Time

Essential Responsibilities: • Conduct comprehensive nutritional assessments of participants to identify their dietary needs and preferences across the continuum, including occasionally visiting home and community settings. • Actively participate in the IDT by offering clinical expertise in team planning, problem-solving, and processes. • Develop and implement individualized nutrition care plans to address participants' health conditions and dietary goals. • Assess nutritional needs, diet restrictions, and current health plans to develop and implement dietary care plans and provide nutritional counseling. • Advise participants and their families on nutritional principles, dietary plans, diet modifications, and food selection and preparation in a manner which honors goal-concordant care. • Provide education and counseling to participants, families, and caregivers on healthy eating habits and nutrition management to improve their quality of life, including but not limited to: diabetes education, heart failure education, renal disease education, and blood glucose and fluid management. • Promote wellness and increase awareness and understanding of self-management. • Inspect meals served for conformance to prescribed diets and standards of palatability and appearance. • Coordinate recipe development, work with food vendors, and standardize and develop new menus for food service operations to meet the needs of participants. • Collaborate closely with other healthcare professionals, including physicians, nurse practitioners, physician associates, nurses, social workers, and therapists, to provide integrated care. • Accurately document in the Electronic Health Record (EHR) in a timely manner Provide all services in a manner consistent with State and Federal regulatory requirements. • Serve on various internal and external committees as required or designated. • Participate in team meetings, caregiver groups, and special events as needed. • Foster a safe and supportive environment conducive to the professional development of healthcare professionals.

Supporting Responsibilities: • Meet department and company performance and attendance expectations. • Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information. • Perform other duties as assigned.

SUCCESS PROFILE

Work Experience: A minimum of 2 years of experience in clinical nutrition, community nutrition, or foodservice management is required. At least 1 year of experience working with geriatric or frail populations, is required. Experience in PACE or other Long-Term Care, Services, and Supports settings is preferred.

Education, Certificates, Licenses: A Bachelor's degree in Dietetics, Nutrition, Food Systems Management, or a related field is required. Candidates must hold an Oregon Licensed Dietician (LD) credential. Certification as a Certified Diabetes Educator (CDE) is preferred. Nationally recognized credentials as a Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) is preferred. A valid CPR certification is required. A valid driver's license is required.
